Alhamdulillah...akhirnya bisa bangun pagi juga buat sholat subuh, setelah beberapa hari kemarin tidurnya selalu dini hari dan berakhir dengan bangun yang kesiangan -__________-

Hari ini rencananya jam 8 am aku dijemput temenku, Echa, buat renang. Aku belum tau pasti dimana tempatnya, katanya di sekitar jalan Paris a.k.a Parangtritis. Tadi malam si Echa cuma kasih liat foto tempat/kolamnya via BBM dan lumayan bagus menurutku. Well, abis dari renang aku rencananya mau ke kampus ngeliat surat ijin penelitianku apakah udah jadi atau belum. Lalu menyiapkan/ngprint lembar kuesioner yang kemaren perlu ditambah beberapa item lagi, lalu sorenya sekitar jam setengah 5 aku udah janjian buat ngadep dokter pembimbing KTIku, drg.Tita R.U.,Sp.Ort. untuk laporan progress penelitianku.

Bismillah ya Allah, lancarkan semua rencanaku ini. Amin...

Referral System on Family Dentistry

Tutorial 4, Scenario 2

Scenario:

A patient came at midnight to the family dentist after experiencing accident that was resulting fracture of 2 front teeth at the lower jaws, intrusion of 1 teeth at upper jaws. Haemorrhage has desisted but the pain at the upper jaws causing the patient suffered and weakend.

The family dentist soon executed emergency treatment by giving anaesthetic to reduce the pain. After seeing the medical record of the patient, the family dentist soon delivered the patient to closest hospital in that area which had established network as centre of vertical reference. The patient condition, therapy which has been done and data of patient in medical record has repasted to his colleague who was in charge in the hospital. The doctor in the hospital soon did required treatment, while the family dentist soon contacted the family of the patient which is recorded in the medical record of the patient. The family dentist informed the case to the patient’s family carefully so they did not feel worry. Patient and his family who has felt believe with the family dentist, requested the family dentist to accompany them when the hospital doctor doing inspection and treatment in the hospital.

Learning Objectives:

1. Jelaskan perbedaan tipe rumah sakit A, B, C, D, dan E, beserta contohnya!

· Rumah Sakit Tipe A

Adalah rumah sakit yang mampu memberikan pelayanan kedokteran spesialis dan subspesialis luas oleh pemerintah ditetapkan sebagai rujukan tertinggi (Top Referral Hospital) atau disebut pula sebagai rumah sakit pusat.

Contoh: RSU Dr Cipto Mangunkusumo, RS PAD Gatot Soebroto, RS Jiwa Jakarta.

· Rumah Sakit Tipe B

Adalah rumah sakit yang mampu memberikan pelayanan kedokteran spesialis dan subspesialis terbatas.Rumah sakit ini didirikan disetiap Ibukota propinsi yang menampung pelayanan rujukan di rumah sakit kabupaten.

- Tipe BII : mempunyai fasilitas dan kemampuan pelayanan medik spesialistik luas dan sub spesialistik terbatas.

Contoh : RS Pusat Pertamina, RS MMC, RSU Persahabatan, RS Jantung Harapan kita, RSPI Prof Dr Sulianti Saroso.

- Tipe B I : Mempunyai fasilitas dan kemampuan pelayanan medik spesialistik sekurang-kurangnya 11 jenis spesialistik.

· Rumah Sakit Tipe C

Adalah rumah sakit yang mapu memberikan pelayanan kedokeran spesialis terbatas.Rumah sakit ini didirikan disetiap ibukota Kabupaten (Regency hospital) yang menampung pelayanan rujukan dari puskesmas.

Contoh: RS medistra,RS UKI cawang, RS Haji Jakarta, RS PAU Antariksa

· Rumah Sakit Tipe D

Adalah rumah sakit yang bersifat transisi dengan kemampuan hanya memberikan pelayanan kedokteran umum dan gigi. Rumah sakit ini menampung rujukan yang berasal dari puskesmas.

Contoh: RSU Gandaria, RSB Asih, RSB Pusdikkes,RS Abdi Waluyo.

· Rumah Sakit Tipe E

Adalah rumah sakit khusus (spesial hospital) yang menyalenggarakan hanya satu macam pelayan kesehatan kedokteran saja.

Contoh: rumah sakit kusta, paru, jantung, kanker, ibu dan anak.

2. Bagaimana hubungan dokter gigi keluarga dengan RS di sekitarnya? Rujukannya harus ke RS pemerintah atau boleh ke swasta?

Dokter gigi keluarga dapat melakukan rujukan ke semua rumah sakit atau pelayanan kesehatan strata lanjut di wilayah kerjanya. Hal ini dimaksudkan agar pelayanan kesehatan kepada pasien dapat dilakukan dengan baik & paripurna.

3. Apakah dokter gigi keluarga boleh menandatangani informed consent pasien?

Inform consent dapat ditandatangani oleh keluarga terdekat pasien. Keluarga terdekat adalah suami atau istri, ayah atau ibu kandung, anak-anak kandung, saudara-saudara kandung atau pengampunya.

Sebelum menerima inform consent untuk ditandatangani, maka dokter harus memberikan penjelasan terlebih dahulu yang meliputi:

a. Diagnosa yang telah ditegakkan.

b. Sifat dan luasnya tindakan yang akan dilakukan.

c. Manfaat dan urgensinya dilakukan tindakan tersebut.

d. Resiko resiko dan komplikasi yang mungkin terjadi daripada tindakan kedokteran tersebut.

e. Konsekwensinya bila tidak dilakukan tindakan tersebut dan adakah alternatif cara pengobatan yang lain.

f. Kadangkala biaya yang menyangkut tindakan kedokteran tersebut.

Pengecualian terhadap keharusan pemberian informasi sebelum dimintakan persetujuan tindakan kedokteran adalah:

a. Dalam keadaan gawat darurat (emergensi), dimana dokter harus segera bertindak untuk menyelamatkan jiwa.

b. Keadaan emosi pasien yang sangat labil sehingga ia tidak bisa menghadapi situasi dirinya.

Ini tercantum dalam PerMenKes no 290/Menkes/Per/III/2008.

Salah satu tugas seorang dokter gigi keluarga dalam kasus rujukan adalah tetap bertanggung-jawab atas pasien yang dirujukan ke Dokter Spesialis atau dirawat di RS, sehingga ketika dalam keadaan emergensi dan keluarga pasien belum dihubungi serta pasien membutuhkan tindakan segera, maka dokter gigi keluarga boleh menandatangani informed consent.

4. Mengapa dokter gigi keluarga membawa rekam medis pasien saat melakukan rujuan (sejauh mana dokter gigi keluarga mengetahui rekam medis pasien)?

Dokter gigi keluarga tidak diperkenankan membawa seluruh dokumen rekam medis milik pasiennya, hanya diperkenankan untuk membawa copy/salinan dari rekam medis dan itu pun yang disalin adalah data-data yang diperlukan untuk hal-hal tertentu (tidak seluruh isi rekam medis disalin).

The Long Holiday

Sometimes, everyone dreams about to spend their holiday while they’re still in their work. But, it’s slightly different when you’re on a holiday, along holiday actually, and you don’t have any plans to spend it for. So bored…

Spend the Holiday in Solo

This afternoon I’ll go to Solo by train. I’ll spend my holiday in my friend’s boarding house for 2 days. And not forget to visit my old friend, too, near the airport. Hope it would be fun!

So Bored…

I’m in my holiday now, actually. I’m going to start the study in September, 9^th so it’s about a month to spend my holiday. But I don’t have any idea what am I suppose to do. I have a plan to go to my friend’s house in Solo at the end of this month, and I also have a plan to have a vacation in Malang with my family (it’s a vacation from my father’s office, they allowed their family member to spend this vacation together, and we’ll go to Malang by bus, in August, 25^th ). So, I don’t have any idea to full fill my ‘vacuum’ days now –__________- I only open my laptop and just chat or browse everything and something else. So bored…

Now I stay in my boarding house, there’re only 3-4 people here

Dinner

Well, today (this early of the evening) I’m gonna to Ambarukmo Plaza, to meet my friend and have a dinner with him. Hopefully the rain doesn’t fall tonight

Hm… I’m a lil’ bit nervous actually, but I don’t know why. Uhm…maybe meet with the other for the first time is such a weird thing for me #Lol

Yiruma

Since I heard my friend’s post about Yiruma on his blog, I love all of his work. Yes, I love musical instrument like piano, saxophone, violin, etc. In my opinion, this kind of music brings us to more relaxed. Even it feels confused (-galau-)

Gathering

Tonight I’m going to have a dinner with my friend, Bayu. He comes from Jakarta, and spend his summer school here in Jogja for about 2 months. So we decided to meet face to face tonight for dinner.

He’s a journalist in OVI. I don’t know for sure what OVI is. So far that I know it is about a non government affiliation (?). We promise that we’ll meet at 5 pm at Ambarukmo Plaza.

Ramadan Day 5

It’s Sunday! And it’s also Ramadan day 5. I’ve just cleaned up my bedroom, wiped all the furniture, swept and also swabbed the floor. After all, I put an aromatherapy of incense stick and got it burned. Hoh…it felt so relaxing

Usually in early evening of Sunday my father asked me to pick him up back to his home in Sleman. But he sent me a message that he’ll back to Sleman tomorrow.

Well, I wish that I could pass this Ramadan well

Annual OSCE and Fasting Day

I’ve just taken a bath and actually after this I’m going to go to campus to put my logbook. But unfortunately it’s almost 12 pm and it means that the staff is taking a rest until 1 pm. So, I decide to delay it.

Well, tomorrow the annual of the OSCE will heal. It takes 2 days, and my friend said that there are about 4 stations on OSCE. The first is about prescription and communication, and the second is about RCT (Root Canal Treatment), the third is about exodontia, and the last is…. taraaa…! I forget!

So I am making a plan today: After I write this blog, I’ll take a prayer first. Then about 1 pm I’ll go to campus to put my logbook and see the test schedule at OSCE’s room. After that, at night, I’ll text Imas (my friend) to study together. Well, hopefully all of those are working!

Oh, tomorrow is also a fasting day. Yep! The Ramadan has come! It means that we’re the Moslem will have a fasting day for a month. Enjoy!

Here I am

‘Bakti Sosial’ at Muhammadiyah Sapen Elementary School

Today I woke up at 5.35 am, and actually I was so unwell due to lack of sleep. Yes, I took a sleep at 2.45 am and I only have about 3 hours to sleep #Yawn After I woke up, I remember that I had to prepare for ‘Bakti Sosial (BakSos)’ (social service (?)), so I took a bath soon. Well, I had this ‘BakSos’ at Muhammadiyah Sapen Elementary School :)


After I finished my bath, I prayed and then prepared the breakfast. Actually, I’d made a boiled noodle first, but I thought it hadn’t any time any longer because it was 6.20 am. I had to gather with the others (my Dental Club friends and Dental Rescue, too) in Asri Medical Centre, not far from my boarding house. Well, then, I drove by motorcycle, and voila…! There was no one there :o

Then, I sent a text message to my friend, asked her if she had already in AMC, too. Fortunately, a few minutes later my friends came there. Then, one of them would buy some meals for the breakfast. So I asked her to buy one for me, too. Next, she came with my order, a bread filled with a Chocolate :p One by one my friends finally came, and at about 7.30 am we were together going to Muhammadiyah Sapen Elementary School.

After we reached there, we went to a room that it would be an operational room. But before we met our patients, we had to have breakfast there (again! :p). Unfortunately I didn't empty my breakfast due to I was so full

Then, after we finished our breakfast, we made a division of tasks. Arina (my friend) and I became an assistant of the Dental Rescue (Dental Rescue is such a Dental Club also but it consists of the students of co-assistant of AMC). Before we made our task, we received a training first, how to took some tools, what should we do with the dental materials, etc.. After that, the patients one by one came to check their teeth. Unfortunately, there were only 4 patients today :(

After the event finished, we got a box of meals for our lunch. Hehe :p Then, we went home after saying goodbye to each other.

So interesting today! :D

The Final Exam…!

Finally, the MCQ #2 in this Soft Tissue Surgery will be healed, tomorrow morning at 7.45 am! Well, hopefully I can do my best and pass all of those exams in this block. Amen

 

Due to this is the last block in this semester, and also the last year of 3^rd  year, after I face with this exams in this block, I still have to face with the yearly exams. Hoh… keep move on!

Multiple Choice Question of 18th Block

Yep, tomorrow (25/6) I’m gonna face with an exam in this block (Soft Tissue Surgery). I’ve done with OSCE examination, now let me concern with the MCQs.

Well, the MCQ will heal in 2 times: MCQ #1 and MCQ #2. Every MCQ has questions in special topics, such as prosthodontia, orthodontia, IRK (Islamic Revealed Knowledge), and so on. Sometimes, the responsibilitor of block gonna tell us what topic that we have learn/focus in each MCQ. Well, it really helps sometimes

Hopefully, I can pass those exams in this block. Wish me luck!

Windows Live Writer

I’ve just configure my blog account on Windows Live Writer. Actually, I downloaded this software when I was downloading Windows Messenger, and voila! It also download Windows Live Writer, Windows Live Family Safety, and also Windows Live Mail (all contained in 1 package).

Before I configure it, I read the tutorial 1^st by searching on Google. The author in some blog said that it still can use even we’re offline (by save your write before you post it), but if you’d post your write on your blog, you’ve to online.

Well, let us see how the Windows Live Writer works!

Gwiyomi Cover Dance

I've just watched about Gwiyomi Dance Cover in YouTube. Actually, I'm not really like Korean dance, sing, or whatever due to I don't know about the language, they just have a cute face, and baby-cute-gesture *I guess*. Well, my BF asked me to make a video too about Gwiyomi. Oh my... should I?? :o

By the way, my BF will come to Jogja next Monday, 3rd of June. Yeay! :p

Bazaar...! :D

Today was so hectic, I think. Due to my class heals a bazaar on campus. It's a work program from my division to make fund raising for social work that will be heal next month. And I have to stand in one corner to sale my friend's stuff :p
Even it was so tired, but I love it. It makes me realize that to find 1 penny's so difficult. So, it makes me to change my mindset that what I have now's so worth :3

Now it's time to take a rest, because this event still heal until until Friday, May 24th.

It's a Satnite, Guys...! Yeay! Full of joy, but also a lil' bit bleak. Yeah, you know, my BF's far far away in Bogor, while I am in Yogyakarta. It's about 16 hours to reach each other by bus. But, well, I take it easy, it doesn't matter for me #CheerUp :D

Well, after I faced with the exam about my proposal several days ago, I still have to fix it. It means, there're a lot of revision in my proposal. From the tittle, the topic, how to write the sentences well, my method, and so many other things that I have to finish. God d*mn! Gimme your help, please... T.T

Tomorrow I'll go to the Malioboro to buy some watch to my BF's birthday present. Well, I hope he would like it :)

Unthinkable

I've just watching a film about terrorism, "Unthinkable" on my laptop. And oh my God, it was really shocking me. Well, you know, that I'm a person who really can't see or hear or whatever about violence. In this film, it's a lot of scenes that contain violence ('till I write this, I still speechless due to what I've just watch about).

MCQ #2, Block of Research Methodology

Tomorrow, at 8.00 am I'll face an exam of MCQ #2, in this block of 17th. Well ya, it means that I have to study hard and study smart than before in order I could pass this exam. Wish me luck, guys! ( ^^)9

And after that, I have to prepare all the things-that-I-have-to-prepare for my presentation. Well, my counselor still doesn't confirm me about when will I have to make a presentation yet. This chance I use to maximize myself to prepare it. Errr... go! go! go! ( ^^)9

Nothing To Do ~(--~)

What a sleepy morning today, hoahm... #Yawn (-.-  )

Well, nothing to do today. Actually, I'm gonna go to the bank to save a 10% of my money 1st (and it has to do!), after that, well yeah, just lie down on my bed (--'!)
I don't have any plan today, just because it's a free week (for me). I mean, I don't have to go to campus to attend on a class, or finish my proposal due to it's submitted to my counselor already. Uhm...maybe I'll make some slides on PowerPoint preparing my final exam of my proposal.

Hah... so lazy today. Someone gimme a job please... (x.X)

Finally, I finish my proposal of my research! Yeay! But, I have to submit it and my mentor check it first, maybe there'll any revision again, fuh...
OK, after this I'll take a bath, then go to the Hospital of Dental and Mouth of Asri Medical Center to submit my proposal to front office (my mentor said: "Just submit it on FO, I'll take it then"). Then prepare for the exam (MCQ) tomorrow. OK, keep fighting...!!! And do not forget to always say:

I've just add a clock, just see it on a right upper side of this blog! Yeay!
Actually, I still don't know how to add some widget such as clock, calendar, who's follow my blog, etc. Sometimes, I search how to manage it by googling, but it doesn't really help me (or maybe due to I'm not really know how to handle it, so all of the suggest on those another-blogs-which-gives-some-tips are useless for me #LOL)

Well, a few minutes ago my BF was sent text message for me, as usual, he always remember me about my draft of my paper. Hiyaaa... ~(--~) OK, fine, next week I'll face my exam, to presentation about my proposal of my research (thesis). Before I face it, I have to finish my proposal first and it must approved by my mentor. So, I only have about 3 days remaining to finish it. Fight! Fight! Fight! (>.<)9
